Our proposed collaborative approach establishes better patient outcomes and an improved working environment for the physician and healthcare facility.
This document addresses the benefit skilled nurse practitioners (NPs) can provide to one of the most vulnerable populations as well as your practice. Studies have demonstrated that NPs provide quality patient care, increase family and patient satisfaction and reduce ER visits and hospital admissions.
•Initial admission H&P
•Annual History & Physicals
•Skilled visits 3-5 times per week
•Regulatory visits alternating with physician every 30-60 days
•Acute visits as needed
•Discharge planning and summary
•Daily review of labs and diagnostics
•Participate in family meetings or care planning as needed
•Coordination of care with interdisciplinary team
•Collaboration with outpatient specialists
•Sign Medicare Certifications and PT/OT/SLP orders
•Fulfill DME requests and controlled substance prescriptions
•Provide End of Life and Palliative Care as well as collaborate with Hospice
•NP cross-coverage for time off
